You half-know the answer already or you would not be here. The short version: "giving head" is slang for oral sex. The longer version is more useful than the dictionary, so keep scrolling.

The Quick Definition

Giving head = using your mouth to stimulate a partner's genitals.

The "head" in the phrase originally referred to the head of the penis, since the tip is the most sensitive part and the etymology of the slang. Today it is used more loosely. "Going down on someone" is essentially the same phrase. So is "blow job" (which despite the name does not involve any actual blowing).

In current usage, the phrase is gender-neutral. People say it about oral sex on women, on men, on anyone. Some prefer specific terms (cunnilingus, fellatio) but in casual speech "head" covers all of it.

The hand-plus-mouth combo

Wrap one hand around the base of your partner. Use your mouth on the upper third, the head specifically. Hand and mouth move together as one unit. Full coverage without gagging. Use lube on the hand, keep it twisting slightly as it moves.
